    1. Renewed annually, need CD Signature, the applicant must include UC Regents, address is the 4-H office, primary contact 4-H staff, alternate-club leader/designee, Hold Harmless clause-needs supplemental UC documentation. Volunteers should not sign any form due to liability.
    2. Apply for “All <name of club> meetings, events and activities, unless specific dates are requested by facility management
    3. Certificate of Insurance- requested by office staff, sent to facility managers and club leaders
    4. FUA hardcopy preferred, online application optional
