About the Survey Registration & Payment Tool
The Survey Registration and Payment tool is a versatile system that can be used to fit many needs. Common uses for the survey tool range from registration forms utilizing Credit Card payments to complex information acquisition forms.
How to access the Survey Registration and Tool Page:
You must be logged into your UCANR Portal. Once there, you can access the survey system in two ways:
- Click the Surveys link in the far-right column of your ANR Portal.
- Log in to your Site Builder 3 site and select the Surveys link from the left navigation menu of your dashboard.
There is one major difference between accessing your survey from the Portal vs. from your Sitebuilder 3 dashboard, that being setting up a Credit Card Survey. You can only set up a credit card survey through the Sitebuilder 3 dashboard and only if your unit or county is authorized to do so.

Before you begin
Before creating a survey, define your goals and determine what information you need to collect. Consider the following questions:
- What is the purpose of your survey?
- How can the survey be simplified while still meeting your objectives?
- How will the survey results be used?
- Are the questions clear and easy for participants to understand?
Begin your survey with a brief description of its purpose and objectives. Then identify the information you need to collect from participants.
Common survey fields include:
- Name
- Location
- Contact Information
Viewing Survey Results
Once your survey has been completed, you can review the results either on your web browser or by downloading an Excel spreadsheet.
